Certificate of Verification Fee: P50.00

Oath Fee: P36.00


How to Avail the Service

Review and Evaluate Request – 15 minutes Chief, FMS
Review and  Assign Forest Ranger to conduct inspection – 15 minutes Chief, Utilization Unit
Conduct actual inspection of the forest products in the area and prepare Inspection Report including Order of Payment – 4 hours to 5 days depending on the location of inspection area and schedule of inspection Forest Ranger/Utilization Unit
Review Inspection Report and Sign Order of Payment – 15 minutes Chief, FMS
 2  Pay Certificate of Verification Fee and Oath Fee Issue Official Receipt (OR) – 15 minutes Bill Collector/Cashier
Prepare and Initial Certificate of Verification – 15 minutes Chief, FMS, Chief
Approve Certificate of Verification – 15 minutes CENRO
Release Certificate of Verification – 15 minutes Releasing Clerk
3 Receive Certificate of Verification Note: FMS personnel shall monitor/check the hauling of the non-wood forest products 1 Day  Receiving Clerk
